Delivery System Reform Incentive Payment Program (“DSRIP”) Care Transition Program
Delivery System Reform Incentive Payment Program’s (“DSRIP”) Care Transition Program – The Foundation’s Care Transition Team, a licensed Social Worker, RN and Outreach Worker, are outstationed and administer the program at Queens Hospital, Jamaica Queens. The goal – to prevent unnecessary re-hospitalizations of Queens Hospital patients by ensuring they are linked to the services they require by their discharge dates, they are compliant with discharge plans for medication and treatment and any needs that emerge within the first 30 days after discharge are expeditiously addressed.
The DSRIP Care Transition Program is made possible by a contract with the New York City Department for the Aging.
